Why COB confuses people
Two plans can both be “real,” but only one is usually primary for a given claim. Providers may bill the wrong plan first, or wait on secondary while sending you a statement.
What to check first

- Which plan name appears as primary on the EOB or bill
- Whether a secondary plan is listed or still pending
- Date of service vs when each plan was active
- Any rejection reason mentioning COB, eligibility, or other coverage
